Medication Reviews at Green Meadow Surgery
Long-term conditions and Month of Birth
We introduced a Month of Birth system for your long-term conditions to be reviewed annually, starting
July 2024. This focuses on patients with certain Long-Term conditions, such as Asthma, Chronic
Obstructive Pulmonary Disease, Heart Failure, Atrial Fibrillation, Hypercholesterolemia, Coronary
Artery Disease, Hypertension, and so on.
The aim is to have an efficient process for reviewing each patient in a planned and proactive way
whilst ensuring we are not doubling up appointments, helping us review more patients in a structured
fashion.
Every single patient on our long-term condition registers gets a full review – –
a Part 1 for blood tests/observations/diabetic foot checks ‘data gathering appointment’
a Part 2 condition and medication review with a clinician, either a pharmacy technician
or a pharmacist to optimise their health and medications
You should be contacted in the month of your Birthday for Part 1, with Part 2 following shortly
after. Any test results taken will be discussed in the Part 2 appointment unless there is a clinical
need to discuss them urgently with a GP sooner.
Non-Long-term Conditions Medication and HRT reviews
We recognise there are patients who may not be on our long-term condition registers but are taking
regular medication prescribed by us. They also need an annual review to ensure the medications are
working and their conditions are optimised to the best of our ability.
We are attempting to review this group in a 2-fold approach. During their month of birth, they will
receive a questionnaire via Accurx – a safe platform that sends your information directly back to a
doctor to review. One of the Partners will review your information. Should you need to have
medication changes or concerns, then we will organise an appointment accordingly to discuss. If any
bloods are needed, such as for your thyroid, then we can request this at the same time too
We are also operating this model for our HRT reviews. A focused questionnaire is sent out to review
your HRT symptoms and medications and if you feel your symptoms are not well controlled then a
review is booked with one of our doctors or pharmacists with an interest in HRT.
Should you feel your condition is well controlled, then upon receiving your response, your medication
is left as it is.
Blood results
Any blood tests that Green Meadows Surgery requests will go back to the Clinician
requesting. The results of these can be seen on the NHS APP within 5 working days, with the
clinician’s comment visible.

We unfortunately are unable to offer routine blood testing appointments at the surgery due to limited
capacity, and those appointments we do have must be kept for urgent clinical need (such as referrals
for suspected cancer or monitoring tests that must be within a certain timeframe).
